The role: At Better Life Partners, you’ll join a virtual care team dedicated to improving outcomes for members with complex medical, behavioral health, and social needs. As a RN Triage Nurse, you’ll play a pivotal role in assessing member needs, coordinating care, and ensuring members connect with the right care at the right time. You’ll work closely with physicians, nurse practitioners, behavioral health clinicians, and clinical operations staff to directly enhance member experience and continuity of care. Unless otherwise specified, this position is remote (work from home). Candidates must reside in one of the following states: Maine, Maryland, Massachusetts, New Hampshire, New Jersey, North Carolina, Pennsylvania, Rhode Island, or Connecticut. What you will do: • Conduct clinical triage for members with substance use disorder and complex medical, behavioral health, and social needs, using evidence-based protocols to assess acuity and determine appropriate level of care • Deliver real-time nursing assessment and clinical decision-making via phone and virtual platforms, serving as a flow facilitator by connecting members to providers based on clinical need • Conduct nursing visits beyond triage, such as initiating visits to support provider workflows and completing follow-arenaflex to ensure coordinated, trauma–informed, harm reduction-based care • Document assessments, triage decisions, and member communications accurately and efficiently in the EHR in compliance with clinical, regulatory, and organizational standards • Collaborate closely with medical providers, behavioral health clinicians, and clinical operations team members to support integrated, team-based care delivery • Apply sound clinical judgment in high-volume, fast-paced environments while maintaining member safety, empathy, and professionalism • Support standardized triage workflows and clinical protocols to ensure consistency, quality, and scalability across a growing virtual care model • Provide guidance to ancillary clinical operations staff, ensuring adherence to policies, procedures, and standards of care You are a good fit if you have: • Active RN license in good standing (multi-state licenses is preferred for NH, ME, MA, or NC; willingness to obtain multi-state licenses is required) • 5+ years of nursing experience; Bachelor’s degree in nursing preferred • Experience providing clinical triage and making independent nursing assessments in ambulatory, virtual, or fast-paced clinical settings • Experience working with complex or underserved populations, including individuals with medical, behavioral, and social complexity • Strong clinical judgment and ability to operate autonomously while collaborating within an interdisciplinary care team • Excellent communication and interpersonal skills, with the ability to engage members empathetically and partner effectively with internal teams • Strong problem-solving skills and adaptability to support timely, member-centered care • Tech-savvy, comfortable working across multiple clinical systems and EHRs, and able to efficiently navigate digital tools and adapt to new technologies It will also be great if you have: • Experience working with individuals with substance use disorder or in harm reduction care models • Experience or interest in workflow optimization and process improvement • Experience working closely with call centers, virtual clinics, or centralized clinical operations teams Work location requirements: • BLP operates in hybrid and remote work environments, which allows us to better meet our members and partners where they’re located. Candidates applying for this role must be willing and able to travel locally within the communities we serve and/or travel to onsite meetings as expected in coordination with their department and business needs. When working from home, the requirements include: • Must have reliable internet service with a fast upload/download ability • Ability to ensure any protected health or proprietary data/information is not visible (or audible) to others in any work location • Must have a quiet space to speak to members, team members, or external partners with minimal background noise and distractions $70,725 - $94,012 a year The actual compensation offered within the range, along with title and level, will be dependent on a variety of factors including, but not limited to, relevant experience, qualifications, skills, level offered, business requirements, and performance expectations.
